Registration Day


Saturday, April 12, 2008 
At SAIT Campus Center, Apollo / Dianna Room.
10:00 am - 1:00 pm    


The fees:

- $200 per swimmer
- $100 for the 3rdchild from the same family


Practices:

Swim practices will start 5:00 pm - 7:00 pm on Monday, May 1, 2007


Forms required:

ASSA Swimmer Registration Form
ASSA Information Privacy Form
Swordfish Registration Form

For registration please remember to bring the following:

  1. A completed 2008 Registration Form with signature on Medical Permission, ASSA Competition Class - Swimmer Registration Form and ASSA FOIP Consent Form
    - one per swimmer
    - make sure Alberta Health Care number must be on the form
  2. Completed cheques
    i.   Cheque for registration fee 
         (1 child = $200; 2 = $400; 3 = $500)
    ii. 3 undated $200 cheque for family commitment
    (2 for meet shifts and 1 for the official's clinic)

    Having the cheques completed ahead of time will make the signup process faster for everyone – your cooperation in this is greatly appreciated!!

    (Please make all cheques payable to CALGARY SWORDFISH SWIM CLUB)
  3. Copy of birth certificate (FOR ALL SWIMMERS)
  4. Your Swimmer: They need to be sized for the club's swim suit. This is included in your fees - it is not an extra cost.
  5. Parents bring your Official’s Card

 

Family Commitment :

  • Attend one official’s training course
  • Officiate at two swim meet
  • Support Swordfish Swim Meet

Official’s Clinic:

We need to compile a complete list of what officials we have in our club. Please bring your official’s card to help us accomplish this. If you don’t have a card, but have been carded for any level, let us know at registration night and we will gladly get a card for you.

If anyone is interested in becoming a carded official there will be an Official’s Clinic held in May (there is no cost to you):

All courses levels will be available with the possible exception of the referee clinic.  Remember that only officials are allowed on deck for the Regional and Provincial meet in August.
